Part of the growing appeal of Motiva® Preservé breast augmentation is its tissue-preserving approach, which enhances breast volume while working in harmony with your natural anatomy. Rather than disrupting key structures, this technique is designed to respect them, resulting in subtle, natural-looking results. However, given that it’s a refined technique designed for select patients, I find it important to clarify what Preservé is and isn’t.
It’s not simply a different implant, and it’s not inherently “better” than traditional breast augmentation. Instead, it represents a distinct philosophy of breast shaping—one that I carefully select based on your anatomy, goals, and the outcome you want to achieve.
As the first board-certified plastic surgeon in Fort Worth to offer the Preservé technique for breast augmentation, here’s how I approach the procedure.
Step 1: Thoughtful Patient Selection and Planning
Every successful cosmetic result begins with careful planning. During your consultation, I evaluate your existing breast tissue, skin quality, and overall proportions to determine whether a tissue-preserving approach is the right fit.
Patients who benefit most from Preservé typically want to enhance volume while maintaining their natural breast shape. If more significant lifting or reshaping is needed, a different technique may better serve you. Whether pursuing breast augmentation, a mommy makeover, or a deep plane facelift, I always guide you toward a treatment plan that delivers the most elegant, balanced result, rather than applying a one-size-fits-all solution.
Step 2: Precise Access Through a Refined Incision
On the day of surgery, I begin with a small, carefully placed incision, most often within the natural crease beneath the breast. This placement allows for discreet healing while giving me precise access to the underlying tissue.
At this stage, precision is everything. Each step is performed with intention, minimizing unnecessary disruption and setting the foundation for a smooth, controlled procedure.
Step 3: Creating the Implant Pocket While Preserving Structure
This is where the Preservé technique truly differs from traditional breast augmentation.
Rather than dissecting through breast tissue, I use specialized instrumentation such as a channel separator to gently develop a pocket along natural tissue planes. This allows me to preserve important structures, such as ligaments, blood supply, and nerve pathways, that contribute to the breast’s natural appearance, feel, and movement.
From a surgical standpoint, this requires a high level of control and anatomical awareness. The goal is not simply to create space, but to do so in a way that maintains the integrity of the surrounding tissue. Using this specialized approach, I’m able to preserve the breast’s natural support structures and deliver an integrated result.
Step 4: Implant Placement With Balance and Precision
Once the pocket is prepared, I gently place the Ergonomix2® SmoothSilk® implant using an insertion sleeve and carefully evaluate its position. The implant is not forced into place; instead, it is guided into a position that works in balance with your existing anatomy.
At this stage, I assess symmetry, contour, and how the breasts will settle over time. Small refinements here can make a meaningful difference. Ultimately, your final result feels cohesive, not artificial.
Step 5: Layered Closure and a Smooth Recovery Experience
After confirming ideal positioning, I close the incision in layers to support proper healing and minimize tension on the skin.
Because the surrounding tissues have been preserved, many patients experience a quicker, more comfortable early recovery and can return to daily activities sooner. That said, this is still breast surgery, and I provide detailed guidance to support a safe, smooth healing process and long-term results you can confidently enjoy for years.
A Refined Approach—For the Right Patient
Preservé reflects an evolution in aesthetic breast surgery that prioritizes tissue preservation when appropriate. It can be an ideal approach for women seeking subtle breast reshaping without a breast lift. At the same time, it’s not the right choice for everyone.
For patients who require lifting, reshaping, or more structural change, traditional breast augmentation techniques remain highly effective and often necessary. Instead of viewing one method as better than the other, I guide you toward the approach that will deliver the most beautiful outcome based on your anatomy, vision, and goals.
